We can't free the tg_pt_gp in core_alua_set_tg_pt_gp_id() because it's
still accessed via configfs. Its release must go through the normal
configfs/refcount process.
The max alua_tg_pt_gps_count check should probably have been done in
core_alua_allocate_tg_pt_gp(), but with the current code userspace could
have created 0x0000ffff + 1 groups, but only set the id for 0x0000ffff.
Then it could have deleted a group with an ID set, and then set the ID for
that extra group and it would work ok.
It's unlikely, but just in case this patch continues to allow that type of
behavior, and just fixes the kfree() while in use bug.

This patch fixes the following bugs:
1. If there are multiple ordered cmds queued and multiple simple cmds
completing, target_restart_delayed_cmds() could be called on different
CPUs and each instance could start a ordered cmd. They could then run in
different orders than they were queued.
2. target_restart_delayed_cmds() and target_handle_task_attr() can race
where:
1. target_handle_task_attr() has passed the simple_cmds == 0 check.
2. transport_complete_task_attr() then decrements simple_cmds to 0.
3. transport_complete_task_attr() runs target_restart_delayed_cmds() and
it does not see any cmds on the delayed_cmd_list.
4. target_handle_task_attr() adds the cmd to the delayed_cmd_list.
The cmd will then end up timing out.
3. If we are sent > 1 ordered cmds and simple_cmds == 0, we can execute
them out of order, because target_handle_task_attr() will hit that
simple_cmds check first and return false for all ordered cmds sent.
4. We run target_restart_delayed_cmds() after every cmd completion, so if
there is more than 1 simple cmd running, we start executing ordered cmds
after that first cmd instead of waiting for all of them to complete.
5. Ordered cmds are not supposed to start until HEAD OF QUEUE and all older
cmds have completed, and not just simple.
6. It's not a bug but it doesn't make sense to take the delayed_cmd_lock
for every cmd completion when ordered cmds are almost never used. Just
replacing that lock with an atomic increases IOPs by up to 10% when
completions are spread over multiple CPUs and there are multiple
sessions/ mqs/thread accessing the same device.
This patch moves the queued delayed handling to a per device work to
serialze the cmd executions for each device and adds a new counter to track
HEAD_OF_QUEUE and SIMPLE cmds. We can then check the new counter to
determine when to run the work on the completion path.

On realtime kernels, spin_lock_irq*(spinlock_t) do not disable the
interrupts, a call to irqs_disabled() will return false thus firing a
warning in __transport_wait_for_tasks().

When attempting to match EXTENDED COPY CSCD descriptors with corresponding
se_devices, target_xcopy_locate_se_dev_e4() currently iterates over LIO's
global devices list which includes all configured backstores.
This change ensures that only initiator-accessible backstores are
considered during CSCD descriptor lookup, according to the session's
se_node_acl LUN list.
To avoid LUN removal race conditions, device pinning is changed from being
configfs based to instead using the se_node_acl lun_ref.

The XCOPY specification in SPC4r37 states that the XCOPY source and
destination device(s) should be derived from the copy source and copy
destination (CSCD) descriptor IDs in the XCOPY segment descriptor.
The CSCD IDs are generally (for block -> block copies), indexes into
the corresponding CSCD descriptor list, e.g.

Currently LIO completely ignores the src and dest CSCD IDs in the
Segment Descriptor List, and instead assumes that the first entry in the
CSCD list corresponds to the source, and the second to the destination.
This commit removes this assumption, by ensuring that the Segment
Descriptor List is parsed prior to processing the CSCD Descriptor List.
CSCD Descriptor List processing is modified to compare the current list
index with the previously obtained src and dest CSCD IDs.
Additionally, XCOPY requests where the src and dest CSCD IDs refer to
the CSCD Descriptor List entry can now be successfully processed.

Check the length of the XCOPY request segment descriptor list against
the value advertised via the MAXIMUM SEGMENT DESCRIPTOR COUNT field in
the RECEIVE COPY OPERATING PARAMETERS response.
spc4r37 6.4.3.5 states:
If the number of segment descriptors exceeds the allowed number, the
copy manager shall terminate the command with CHECK CONDITION status,
with the sense key set to ILLEGAL REQUEST, and the additional sense
code set to TOO MANY SEGMENT DESCRIPTORS.
This functionality is testable using the libiscsi
ExtendedCopy.DescrLimits test.

Maurizio found a race where the abort and cmd stop paths can race as
follows:
1. thread1 runs iscsit_release_commands_from_conn and sets
CMD_T_FABRIC_STOP.
2. thread2 runs iscsit_aborted_task and then does __iscsit_free_cmd. It
then returns from the aborted_task callout and we finish
target_handle_abort and do:
target_handle_abort -> transport_cmd_check_stop_to_fabric ->
lio_check_stop_free -> target_put_sess_cmd
The cmd is now freed.
3. thread1 now finishes iscsit_release_commands_from_conn and runs
iscsit_free_cmd while accessing a command we just released.
In __target_check_io_state we check for CMD_T_FABRIC_STOP and set the
CMD_T_ABORTED if the driver is not cleaning up the cmd because of a session
shutdown. However, iscsit_release_commands_from_conn only sets the
CMD_T_FABRIC_STOP and does not check to see if the abort path has claimed
completion ownership of the command.
This adds a check in iscsit_release_commands_from_conn so only the abort or
fabric stop path cleanup the command.

This can be reproduced via the following steps:
1. Initiator A tries to log in to iqn1-tpg1 on port 3260. After finishing
PDU exchange in the login thread and before the negotiation is finished
the the network link goes down. At this point A has not finished login
and tpg->np_login_sem is held.
2. Initiator B tries to log in to iqn2-tpg1 on port 3260. After finishing
PDU exchange in the login thread the target expects to process remaining
login PDUs in workqueue context.
3. Initiator A' tries to log in to iqn1-tpg1 on port 3260 from a new
socket. A' will wait for tpg->np_login_sem with np->np_login_timer
loaded to wait for at most 15 seconds. The lock is held by A so A'
eventually times out.
4. Before A' got timeout initiator B gets negotiation failed and calls
iscsi_target_login_drop()->iscsi_target_login_sess_out(). The
np->np_login_timer is canceled and initiator A' will hang forever.
Because A' is now in the login thread, no new login requests can be
serviced.
Fix this by moving iscsi_stop_login_thread_timer() out of
iscsi_target_login_sess_out(). Also remove iscsi_np parameter from
iscsi_target_login_sess_out().

If tcmu_handle_completions() has to process a padding shorter than
sizeof(struct tcmu_cmd_entry), the current call to
tcmu_flush_dcache_range() with sizeof(struct tcmu_cmd_entry) as length
param is wrong and causes crashes on e.g. ARM, because
tcmu_flush_dcache_range() in this case calls
flush_dcache_page(vmalloc_to_page(start)); with start being an invalid
address above the end of the vmalloc'ed area.
The fix is to use the minimum of remaining ring space and sizeof(struct
tcmu_cmd_entry) as the length param.
The patch was tested on kernel 4.19.118.

Creation of the response to READ FULL STATUS fails for FC based
reservations. Reason is the too high loop limit (< 24) in
fc_get_pr_transport_id(). The string representation of FC WWPN is 23 chars
long only ("11:22:33:44:55:66:77:88"). So when i is 23, the loop body is
executed a last time for the ending '\0' of the string and thus hex2bin()
reports an error.

iscsit_close_connection() calls isert_wait_conn(). Due to commit
e9d3009cb936 both functions call target_wait_for_sess_cmds() although that

Where A1,A2... are proposed algorithms, in order of preference.

For the Algorithm, as stated in [RFC1994], one value is required to
be implemented:
5 (CHAP with MD5)
LIO currently checks for this value by only comparing a single byte in
the tokenized Algorithm string, which means that any value starting with
a '5' (e.g. "55") is interpreted as "CHAP with MD5". Fix this by
comparing the entire tokenized string.

passthrough_parse_cdb() - used by TCMU and PSCSI - attepts to reset the LUN
field of SCSI-2 CDBs (bits 5,6,7 of byte 1). The current code is wrong as
for newer commands not having the LUN field it overwrites relevant command
bits (e.g. for SECURITY PROTOCOL IN / OUT). We think this code was
unnecessary from the beginning or at least it is no longer useful. So we
remove it entirely.

4.3.1 ASCII data field requirements
ASCII data fields shall contain only ASCII printable characters (i.e.,
code values 20h to 7Eh) and may be terminated with one or more ASCII null
(00h) characters. ASCII data fields described as being left-aligned
shall have any unused bytes at the end of the field (i.e., highest
offset) and the unused bytes shall be filled with ASCII space characters
(20h).
LIO currently space-pads the T10 VENDOR IDENTIFICATION and PRODUCT
IDENTIFICATION fields in the standard INQUIRY data. However, the PRODUCT
REVISION LEVEL field in the standard INQUIRY data as well as the T10 VENDOR
IDENTIFICATION field in the INQUIRY Device Identification VPD Page are
zero-terminated/zero-padded.
Fix this inconsistency by using space-padding for all of the above fields.

This change has the following effects, in order of descreasing importance:
1) Prevent a stack buffer overflow
2) Do not append an unnecessary NULL to an anyway binary buffer, which
is writing one byte past client_digest when caller is:
chap_string_to_hex(client_digest, chap_r, strlen(chap_r));
The latter was found by KASAN (see below) when input value hes expected size
(32 hex chars), and further analysis revealed a stack buffer overflow can
happen when network-received value is longer, allowing an unauthenticated
remote attacker to smash up to 17 bytes after destination buffer (16 bytes
attacker-controlled and one null). As switching to hex2bin requires
specifying destination buffer length, and does not internally append any null,
it solves both issues.
This addresses CVE-2018-14633.
Beyond this:
- Validate received value length and check hex2bin accepted the input, to log
this rejection reason instead of just failing authentication.
- Only log received CHAP_R and CHAP_C values once they passed sanity checks.

The problem is that iscsi_login_zero_tsih_s1 sets conn->sess early in
iscsi_login_set_conn_values. If the function fails later like when we
alloc the idr it does kfree(sess) and leaves the conn->sess pointer set.
iscsi_login_zero_tsih_s1 then returns -Exyz and we then call
iscsi_target_login_sess_out and access the freed memory.
This patch has iscsi_login_zero_tsih_s1 either completely setup the
session or completely tear it down, so later in
iscsi_target_login_sess_out we can just check for it being set to the
connection.

If we failed to read data from backing file (probably because some one
truncate file under us), we must zerofill cmd's data, otherwise it will
be returned as is. Most likely cmd's data are unitialized pages from
page cache. This result in information leak.

The uio_mem structure has a member that is a phys_addr_t, but can
be a number of other types too. The target core driver attempts
to assign a pointer from vmalloc() to it, by casting it to
phys_addr_t, but that causes a warning when phys_addr_t is longer
than a pointer:

This adds another cast to uintptr_t to shut up the warning.

This patch fixes bug where early se_cmd exceptions that occur
before backend execution can result in use-after-free if/when
a subsequent ABORT_TASK occurs for the same tag.
Since an early se_cmd exception will have had se_cmd added to
se_session->sess_cmd_list via target_get_sess_cmd(), it will
not have CMD_T_COMPLETE set by the usual target_complete_cmd()
backend completion path.
This causes a subsequent ABORT_TASK + __target_check_io_state()
to signal ABORT_TASK should proceed. As core_tmr_abort_task()
executes, it will bring the outstanding se_cmd->cmd_kref count
down to zero releasing se_cmd, after se_cmd has already been
queued with error status into fabric driver response path code.
To address this bug, introduce a CMD_T_PRE_EXECUTE bit that is
set at target_get_sess_cmd() time, and cleared immediately before
backend driver dispatch in target_execute_cmd() once CMD_T_ACTIVE
is set.
Then, check CMD_T_PRE_EXECUTE within __target_check_io_state() to
determine when an early exception has occured, and avoid aborting
this se_cmd since it will have already been queued into fabric
driver response path code.

Since commit 59b6986dbf fixed a potential NULL pointer dereference
by allocating a se_tmr_req for ISCSI_TM_FUNC_TASK_REASSIGN, the
se_tmr_req is currently leaked by iscsit_free_cmd() because no
iscsi_cmd->se_cmd.se_tfo was associated.
To address this, treat ISCSI_TM_FUNC_TASK_REASSIGN like any other
TMR and call transport_init_se_cmd() + target_get_sess_cmd() to
setup iscsi_cmd->se_cmd.se_tfo with se_cmd->cmd_kref of 2.
This will ensure normal release operation once se_cmd->cmd_kref
reaches zero and target_release_cmd_kref() is invoked, se_tmr_req
will be released via existing target_free_cmd_mem() and
core_tmr_release_req() code.

This fixes the following races:
1. core_alua_do_transition_tg_pt could have read
tg_pt_gp_alua_access_state and gone into this if chunk:
if (!explicit &&
atomic_read(&tg_pt_gp->tg_pt_gp_alua_access_state) ==
ALUA_ACCESS_STATE_TRANSITION) {
and then core_alua_do_transition_tg_pt_work could update the
state. core_alua_do_transition_tg_pt would then only set
tg_pt_gp_alua_pending_state and the tg_pt_gp_alua_access_state would
not get updated with the second calls state.
2. core_alua_do_transition_tg_pt could be setting
tg_pt_gp_transition_complete while the tg_pt_gp_transition_work
is already completing. core_alua_do_transition_tg_pt then waits on the
completion that will never be called.
To handle these issues, we just call flush_work which will return when
core_alua_do_transition_tg_pt_work has completed so there is no need
to do the complete/wait. And, if core_alua_do_transition_tg_pt_work
was running, instead of trying to sneak in the state change, we just
schedule up another core_alua_do_transition_tg_pt_work call.
Note that this does not handle a possible race where there are multiple
threads call core_alua_do_transition_tg_pt at the same time. I think
we need a mutex in target_tg_pt_gp_alua_access_state_store.

The implicit transition time tells initiators the min time
to wait before timing out a transition. We currently schedule
the transition to occur in tg_pt_gp_implicit_trans_secs
seconds so there is no room for delays. If
core_alua_do_transition_tg_pt_work->core_alua_update_tpg_primary_metadata
needs to write out info to a remote file, then the initiator can
easily time out the operation.

If tcmu-runner is processing a STPG and needs to change the kernel's
ALUA state then we cannot use the same work queue for task management
requests and ALUA transitions, because we could deadlock. The problem
occurs when a STPG times out before tcmu-runner is able to
call into target_tg_pt_gp_alua_access_state_store->
core_alua_do_port_transition -> core_alua_do_transition_tg_pt ->
queue_work. In this case, the tmr is on the work queue waiting for
the STPG to complete, but the STPG transition is now queued behind
the waiting tmr.
Note:
This bug will also be fixed by this patch:
http://www.spinics.net/lists/target-devel/msg14560.html
which switches the tmr code to use the system workqueues.
For both, I am not sure if we need a dedicated workqueue since
it is not a performance path and I do not think we need WQ_MEM_RECLAIM
to make forward progress to free up memory like the block layer does.

This patch fixes a bug during QUEUE_FULL where transport_complete_qf()
calls transport_complete_task_attr() after it's already been invoked
by target_complete_ok_work() or transport_generic_request_failure()
during initial completion, preceeding QUEUE_FULL.
This will result in se_device->simple_cmds, se_device->dev_cur_ordered_id
and/or se_device->dev_ordered_sync being updated multiple times for
a single se_cmd.
To address this bug, clear SCF_TASK_ATTR_SET after the first call
to transport_complete_task_attr(), and avoid updating SCSI task
attribute related counters for any subsequent calls.
Also, when a se_cmd is deferred due to ordered tags and executed
via target_restart_delayed_cmds(), set CMD_T_SENT before execution
matching what target_execute_cmd() does.

occur when a non immediate TMR is proceeded our of command
sequence number order, and CMDSN_LOWER_THAN_EXP is returned

special case following what iscsit_process_scsi_cmd() does
upon CMDSN_LOWER_THAN_EXP.

However, list_del_init() usage is required during the special

followed by target_complete_nacl() doing the same thing.

In case of unsolicited data for the first sequence
seq_end_offset must be set to minimum of total data length
and FirstBurstLength, so do not add cmd->write_data_done
to the min of total data length and FirstBurstLength.
This patch avoids that with ImmediateData=Yes, InitialR2T=No,
MaxXmitDataSegmentLength < FirstBurstLength that a WRITE command
with IO size above FirstBurstLength triggers sequence error

It would happen because each iscsit_reset_np_thread() sets state
to ISCSI_NP_THREAD_RESET, sends SIGINT, and then blocks waiting
for completion on iscsi_np->np_restart_comp.
However, if iscsi_np was active processing a login request and
more than a single iscsit_reset_np_thread() caller to the same
iscsi_np was blocked on iscsi_np->np_restart_comp, iscsi_np
kthread process context in __iscsi_target_login_thread() would
flush pending signals and only perform a single completion of
np->np_restart_comp before going back to sleep within transport
specific iscsit_transport->iscsi_accept_np code.
To address this bug, add a iscsi_np->np_reset_count and update
__iscsi_target_login_thread() to keep completing np->np_restart_comp
until ->np_reset_count has reached zero.

On receiving text request iscsi-target allocates buffer for
payload in iscsit_handle_text_cmd() and assigns buffer pointer
to cmd->text_in_ptr, this buffer is currently freed in
iscsit_release_cmd(), if iscsi-target sets 'C' bit in text
response then it will receive another text request from the
initiator with ttt != 0xffffffff in this case iscsi-target
will find cmd using itt and call iscsit_setup_text_cmd()
which will set cmd->text_in_ptr to NULL without freeing
previously allocated buffer.
This patch fixes this issue by calling kfree(cmd->text_in_ptr)
in iscsit_setup_text_cmd() before assigning NULL to it.
For the first text request cmd->text_in_ptr is NULL as
cmd is memset to 0 in iscsit_allocate_cmd().

Normally when explicit logout attempt fails, the tx thread context
exits and iscsit_close_connection() from rx thread context does the
extra cleanup once it detects conn->conn_logout_remove has not been
cleared by the logout type specific post handlers.
To address this special case, if the logout post handler in tx thread
context detects conn->tx_thread_active has already been cleared, simply
return and exit in order for existing iscsit_close_connection()
logic from rx thread context do failed logout cleanup.

iscsi-target: Add sk->sk_state_change to cleanup after TCP failure
which would trigger a NULL pointer dereference when a TCP connection
was closed asynchronously via iscsi_target_sk_state_change(), but only
when the initial PDU processing in iscsi_target_do_login() from iscsi_np
process context was blocked waiting for backend I/O to complete.
To address this issue, this patch makes the following changes.
First, it introduces some common helper functions used for checking
socket closing state, checking login_flags, and atomically checking
socket closing state + setting login_flags.
Second, it introduces a LOGIN_FLAGS_INITIAL_PDU bit to know when a TCP
connection has dropped via iscsi_target_sk_state_change(), but the
initial PDU processing within iscsi_target_do_login() in iscsi_np
context is still running. For this case, it sets LOGIN_FLAGS_CLOSED,
but doesn't invoke schedule_delayed_work().
The original NULL pointer dereference case reported by MNC is now handled
by iscsi_target_do_login() doing a iscsi_target_sk_check_close() before
transitioning to FFP to determine when the socket has already closed,
or iscsi_target_start_negotiation() if the login needs to exchange
more PDUs (eg: iscsi_target_do_login returned 0) but the socket has
closed. For both of these cases, the cleanup up of remaining connection
resources will occur in iscsi_target_start_negotiation() from iscsi_np
process context once the failure is detected.
Finally, to handle to case where iscsi_target_sk_state_change() is
called after the initial PDU procesing is complete, it now invokes
conn->login_work -> iscsi_target_do_login_rx() to perform cleanup once
existing iscsi_target_sk_check_close() checks detect connection failure.
For this case, the cleanup of remaining connection resources will occur
in iscsi_target_do_login_rx() from delayed workqueue process context
once the failure is detected.
