If a vb2_queue sets q->min_buffers_needed then when the number of
queued buffers reaches q->min_buffers_needed, vb2_core_qbuf() will call
the start_streaming() callback. If start_streaming() returns an error,
then that error was just returned by vb2_core_qbuf(), but the buffer
was still queued. However, userspace expects that if VIDIOC_QBUF fails,
the buffer is returned dequeued.
So if start_streaming() fails, then remove the buffer from the queue,
thus avoiding this unwanted side-effect.

When an IOCTL with argument size larger than 128 that also used array
arguments were handled, two memory allocations were made but alas, only
the latter one of them was released. This happened because there was only
a single local variable to hold such a temporary allocation.
Fix this by adding separate variables to hold the pointers to the
temporary allocations.

Control events can leak kernel memory since they do not fully zero the
event. The same code is present in both v4l2-ctrls.c and uvc_ctrl.c, so
fix both.
It appears that all other event code is properly zeroing the structure,
it's these two places.

Userspace shouldn't set bytesused to 0 for output buffers.
vb2_warn_zero_bytesused() warns about this (only once!), but it also
calls WARN_ON(1), which is confusing since it is not immediately clear
that it warns about a 0 value for bytesused.
Just drop the WARN_ON as it serves no purpose.

If a filehandle is dup()ped, then it is possible to close it from one fd
and call mmap from the other. This creates a race condition in vb2_mmap
where it is using queue data that __vb2_queue_free (called from close())
is in the process of releasing.
By moving up the mutex_lock(mmap_lock) in vb2_mmap this race is avoided
since __vb2_queue_free is called with the same mutex locked. So vb2_mmap
now reads consistent buffer data.

them with 'gup_flags' to make the use of FOLL_FORCE explicit in callers
as use of this flag can result in surprising behaviour (and hence bugs)

Patch ad608fbcf166 changed how events were subscribed to address an issue
elsewhere. As a side effect of that change, the "add" callback was called
before the event subscription was added to the list of subscribed events,
causing the first event queued by the add callback (and possibly other
events arriving soon afterwards) to be lost.
Fix this by adding the subscription to the list before calling the "add"
callback, and clean up afterwards if that fails.

This commit fixes a bug in upstream commit a136f59c0a1f ("vb2: Move
buffer cache synchronisation to prepare from queue") which isn't
present in 4.4.

The event subscriptions are added to the subscribed event list while
holding a spinlock, but that lock is subsequently released while still
accessing the subscription object. This makes it possible to unsubscribe
the event --- and freeing the subscription object's memory --- while
the subscription object is simultaneously accessed.
Prevent this by adding a mutex to serialise the event subscription and
unsubscription. This also gives a guarantee to the callback ops that the
add op has returned before the del op is called.
This change also results in making the elems field less special:
subscriptions are only added to the event list once they are fully
initialised.

The vb2_core_qbuf() function didn't check if q->error was set. It is
checked in __buf_prepare(), but that function isn't called if the buffer
was already prepared before with VIDIOC_PREPARE_BUF.
So check it at the start of vb2_core_qbuf() as well.

When a buffer is queued or requeued in vb2_buffer_done, then don't
call the finish memop. In this case the buffer is only returned to vb2,
not to userspace.
Calling 'finish' here will cause an unbalance when the queue is
canceled, since the core will call the same memop again.

em28xx selects VIDEO_TUNER, which has a dependency on MEDIA_TUNER,
so we get a Kconfig warning if that is disabled:

VIDEO_TUNER does not actually depend on MEDIA_TUNER, and the
dependency does nothing except cause the above warning, so let's
remove it.

The 32-bit compat v4l2 ioctl handling is implemented based on its 64-bit
equivalent. It converts 32-bit data structures into its 64-bit
equivalents and needs to provide the data to the 64-bit ioctl in user
space memory which is commonly allocated using
compat_alloc_user_space().
However, due to how that function is implemented, it can only be called
a single time for every syscall invocation.
Supposedly to avoid this limitation, the existing code uses a mix of
memory from the kernel stack and memory allocated through
compat_alloc_user_space().
Under normal circumstances, this would not work, because the 64-bit
ioctl expects all pointers to point to user space memory. As a
workaround, set_fs(KERNEL_DS) is called to temporarily disable this
extra safety check and allow kernel pointers. However, this might
introduce a security vulnerability: The result of the 32-bit to 64-bit
conversion is writeable by user space because the output buffer has been
allocated via compat_alloc_user_space(). A malicious user space process
could then manipulate pointers inside this output buffer, and due to the
previous set_fs(KERNEL_DS) call, functions like get_user() or put_user()
no longer prevent kernel memory access.
The new approach is to pre-calculate the total amount of user space
memory that is needed, allocate it using compat_alloc_user_space() and
then divide up the allocated memory to accommodate all data structures
that need to be converted.
An alternative approach would have been to retain the union type karg
that they allocated on the kernel stack in do_video_ioctl(), copy all
data from user space into karg and then back to user space. However, we
decided against this approach because it does not align with other
compat syscall implementations. Instead, we tried to replicate the
get_user/put_user pairs as found in other places in the kernel:

Clearly nobody could be bothered to upstream this patch or at minimum
tell us :-( We only heard about this a week ago.
This patch was rebased and cleaned up. Compared to the original I
also swapped the order of the convert_in_user arguments so that they
matched copy_in_user. It was hard to review otherwise. I also replaced
the ALLOC_USER_SPACE/ALLOC_AND_GET by a normal function.

If the device is of type VFL_TYPE_SUBDEV then vdev->ioctl_ops
is NULL so the 'if (!ops->vidioc_query_ext_ctrl)' check would crash.
Add a test for !ops to the condition.
All sub-devices that have controls will use the control framework,
so they do not have an equivalent to ops->vidioc_query_ext_ctrl.
Returning false if ops is NULL is the correct thing to do here.

ctrl_is_pointer just hardcoded two known string controls, but that
caused problems when using e.g. custom controls that use a pointer
for the payload.
Reimplement this function: it now finds the v4l2_ctrl (if the driver
uses the control framework) or it calls vidioc_query_ext_ctrl (if the
driver implements that directly).
In both cases it can now check if the control is a pointer control
or not.

Instead of doing sizeof(struct foo) use sizeof(*up). There even were
cases where 4 * sizeof(__u32) was used instead of sizeof(kp->reserved),
which is very dangerous when the size of the reserved array changes.
