staging: octeon-usb: cvmx-usb: fix whitespace issues
Fix most of the whitespace issues reported by checkpatch:
- use tabs for indentation
- brace placement vs. newlines
- (foo_t*) -> (foo_t *)
- i=0 -> i = 0
