USB control and bulk message timeouts are specified in milliseconds and
should specifically not vary with CONFIG_HZ.

Patch fixes 6fire not to use stack as URB transfer_buffer. URB buffers need to
be DMA-able, which stack is not. Furthermore, transfer_buffer should not be
allocated as part of larger device structure because DMA coherency issues and
patch fixes this issue too.

CONFIG_HOTPLUG is going away as an option. As result the __dev*
markings will be going away.
Remove use of __devinit, __devexit_p, __devinitdata, __devinitconst,
and __devexit.

- Hardware Master volume
- PCM 44-192kHz@24 bits, 6 channels out, 4 channels in (analog)
- MIDI in/out
- firmware loading after cold start
- phono/line switching
