task_is_dead() has no users since commit 43e13cc107cf
("cred: remove task_is_dead() from __task_cred() validation"), and
nobody except exit.c should rely on ->exit_state (we still have
the users which should be changed).

This patch fixes a problem with the initialization of the
struct perf_event active_entry field. It is defined inside
an anonymous union and was initialized in perf_event_alloc()
using INIT_LIST_HEAD(). However at that time, we do not know
whether the event is going to use active_entry or hlist_entry (SW).
Or at last, we don't want to make that determination there.
The problem is that hlist and list_head are not initialized
the same way. One is okay with NULL (from kzmalloc), the other
needs to pointers to point to self.
This patch resolves this problem by dropping the union.
This will avoid problems later on, if someone starts using
active_entry or hlist_entry without verifying that they
actually overlap. This also solves the initialization
problem.

This patch adds a new field to the struct perf_event.
It is intended to be used to chain events which are
active (enabled). It helps in the hardware layer
for PMUs which do not have actual counter restrictions, i.e.,
free running read-only counters. Active events are chained
as opposed to being tracked via the counter they use.
To save space we use a union with hlist_entry as both
are mutually exclusive (suggested by Jiri Olsa).

1. Don't include asm/uprobes.h unconditionally, we only need
it if CONFIG_UPROBES.
2. Move the definition of "struct xol_area" into uprobes.c.
Perhaps we should simply kill struct uprobes_state, it buys
nothing.
3. Kill the dummy definition of uprobe_get_swbp_addr(), nobody
except handle_swbp() needs it.
4. Purely cosmetic, but move the decl of uprobe_get_swbp_addr()
up, close to other __weak helpers.

uprobe_task->vaddr is a bit strange. The generic code uses it only
to pass the additional argument to arch_uprobe_pre_xol(), and since
it is always equal to instruction_pointer() this looks even more
strange.
And both utask->vaddr and and utask->autask have the same scope,
they only have the meaning when the task executes the probed insn
out-of-line, so it is safe to reuse both in UTASK_RUNNING state.
This all means that logically ->vaddr belongs to arch_uprobe_task
and we should probably move it there, arch_uprobe_pre_xol() can
record instruction_pointer() itself.
OTOH, it is also used by uprobe_copy_process() and dup_xol_work()
for another purpose, this doesn't look clean and doesn't allow to
move this member into arch_uprobe_task.
This patch adds the union with 2 anonymous structs into uprobe_task.
The first struct is autask + vaddr, this way we "almost" move vaddr
into autask.
The second struct has 2 new members for uprobe_copy_process() paths:
->dup_xol_addr which can be used instead ->vaddr, and ->dup_xol_work
which can be used to avoid kmalloc() and simplify the code.
Note that this union will likely have another member(s), we need
something like "private_data_for_handlers" so that the tracing
handlers could use it to communicate with call_fetch() methods.

There is currently no way to initialize a global RCU-protected pointer
without either putting up with sparse complaints or open-coding an
obscure cast. This commit therefore creates RCU_INITIALIZER(), which
is intended to be used as follows:
struct foo __rcu *p = RCU_INITIALIZER(&my_rcu_structure);
This commit also applies RCU_INITIALIZER() to eliminate repeated
open-coded obscure casts in __rcu_assign_pointer(), RCU_INIT_POINTER(),
and RCU_POINTER_INITIALIZER(). This commit also inlines
__rcu_assign_pointer() into its only caller, rcu_assign_pointer().

The rcu_assign_pointer() primitive needs to use ACCESS_ONCE to make
the assignment to the destination pointer volatile, to protect against
compilers too clever for their own good.

The rcu_assign_pointer() macro, as with most cpp macros, must not evaluate
its argument more than once. And it in fact does not. But this might
not be obvious to the casual observer, because one of the arguments
appears no less than three times. However, but one expansion is only
visible to sparse (__CHECKER__), and one lives inside a typeof (where
it will never be evaluated), so this is in fact safe.
This commit therefore adds a comment making this explicit.

Function prototypes don't need to have the "extern" keyword since this
is the default behavior. Its explicit use is redundant. This commit
therefore removes them.

Currently blocking in an RCU callback function will result in
"scheduling while atomic", which could be triggered for any number
of reasons. To aid debugging, this patch introduces a rcu_callback_map
that is used to tie the inappropriate voluntary context switch back
to the fact that the function is being invoked from within a callback.

A number of situations currently require the heavyweight smp_mb(),
even though there is no need to order prior stores against later
loads. Many architectures have much cheaper ways to handle these
situations, but the Linux kernel currently has no portable way
to make use of them.
This commit therefore supplies smp_load_acquire() and
smp_store_release() to remedy this situation. The new
smp_load_acquire() primitive orders the specified load against
any subsequent reads or writes, while the new smp_store_release()
primitive orders the specifed store against any prior reads or
writes. These primitives allow array-based circular FIFOs to be
implemented without an smp_mb(), and also allow a theoretical
hole in rcu_assign_pointer() to be closed at no additional
expense on most architectures.
In addition, the RCU experience transitioning from explicit
smp_read_barrier_depends() and smp_wmb() to rcu_dereference()
and rcu_assign_pointer(), respectively resulted in substantial
improvements in readability. It therefore seems likely that
replacing other explicit barriers with smp_load_acquire() and
smp_store_release() will provide similar benefits. It appears
that roughly half of the explicit barriers in core kernel code
might be so replaced.

The Linux kernel has traditionally required that an UNLOCK+LOCK
pair act as a full memory barrier when either (1) that
UNLOCK+LOCK pair was executed by the same CPU or task, or (2)
the same lock variable was used for the UNLOCK and LOCK. It now
seems likely that very few places in the kernel rely on this
full-memory-barrier semantic, and with the advent of queued
locks, providing this semantic either requires complex
reasoning, or for some architectures, added overhead.
This commit therefore adds a smp_mb__after_unlock_lock(), which
may be placed after a LOCK primitive to restore the
full-memory-barrier semantic. All definitions are currently
no-ops, but will be upgraded for some architectures when queued
locks arrive.

"Currently there are two methods to set the panic_timeout: via
'panic=X' boot commandline option, or via /proc/sys/kernel/panic.
This tree adds a third panic_timeout configuration method:
configuration via Kconfig, via CONFIG_PANIC_TIMEOUT=X - useful to
distros that generally want their kernel defaults to come with the
.config.
CONFIG_PANIC_TIMEOUT defaults to 0, which was the previous default
value of panic_timeout.
Doing that unearthed a few arch trickeries regarding arch-special
panic_timeout values and related complications - hopefully all
resolved to the satisfaction of everyone"

The panic_timeout value can be set via the command line option
'panic=x', or via /proc/sys/kernel/panic, however that is not
sufficient when the panic occurs before we are able to set up
these values. Thus, add a CONFIG_PANIC_TIMEOUT so that we can
set the desired value from the .config.
The default panic_timeout value continues to be 0 - wait
forever. Also adds set_arch_panic_timeout(new_timeout,
arch_default_timeout), which is intended to be used by arches in
arch_setup(). The idea being that the new_timeout is only set if
the user hasn't changed from the arch_default_timeout.

Commit 436d42c61c3e ("ARM: samsung: move platform_data definitions")
moved the file to the current location but forgot to remove the pointer
to its previous location. Clean it up. While at it also change the header

The Zorro definitions and device IDs are used by bootstraps, hence they
should be exported through UAPI.
Unfortunately zorro.h was never marked for export when headers_install
was introduced, so it was forgotten during the big UAPI disintegration.
In addition, the removal of zorro_ids.h had been sneaked into commit
7e7a43c32a8970ea2bfc3d1af353dcb1a9237769 ("PCI: don't export device IDs to
userspace") before, so it was also forgotten.
Split off and export the Zorro definitions used by bootstraps.

Currently the array of Zorro devices is allocated statically, wasting
up to 4.5 KiB when running an Amiga or multi-platform kernel on a machine
with no or a handful of Zorro expansion cards. Convert it to conditional
dynamic memory allocation to fix this.
amiga_parse_bootinfo() still needs to store some information about the
detected Zorro devices, at a time even the bootmem allocator is not yet
available. This is now handled using a much smaller array (typically less
than 0.5 KiB), which is __initdata and thus freed later.

Linus disliked the _no_lockdep() naming, so instead
use the more-consistent raw_* prefix to the non-lockdep
enabled seqcount methods.
This also adds raw_ methods for the write operations
as well, which will be utilized in a following patch.

The body of i2c_parent_is_i2c_adapter() is currently guarded by
I2C_MUX. It should be CONFIG_I2C_MUX instead.
Among potentially other problems, this resulted in i2c_lock_adapter()
only locking I2C mux child adapters, and not the parent adapter. In
turn, this could allow inter-mingling of mux child selection and I2C
transactions, which could result in I2C transactions being directed to
the wrong I2C bus, and possibly even switching between busses in the
middle of a transaction.
One concrete issue caused by this bug was corrupted HDMI EDID reads
during boot on the NVIDIA Tegra Seaboard system, although this only
became apparent in recent linux-next, when the boot timing was changed
just enough to trigger the race condition.

Currently, the tx queue were selected implicitly in ndo_dfwd_start_xmit(). The
will cause several issues:
- NETIF_F_LLTX were removed for macvlan, so txq lock were done for macvlan
instead of lower device which misses the necessary txq synchronization for
lower device such as txq stopping or frozen required by dev watchdog or
control path.
- dev_hard_start_xmit() was called with NULL txq which bypasses the net device
watchdog.
- dev_hard_start_xmit() does not check txq everywhere which will lead a crash
when tso is disabled for lower device.
Fix this by explicitly introducing a new param for .ndo_select_queue() for just
selecting queues in the case of l2 forwarding offload. netdev_pick_tx() was also
extended to accept this parameter and dev_queue_xmit_accel() was used to do l2
forwarding transmission.
With this fixes, NETIF_F_LLTX could be preserved for macvlan and there's no need
to check txq against NULL in dev_hard_start_xmit(). Also there's no need to keep
a dedicated ndo_dfwd_start_xmit() and we can just reuse the code of
dev_queue_xmit() to do the transmission.
In the future, it was also required for macvtap l2 forwarding support since it
provides a necessary synchronization method.

VM to VM GSO traffic is broken if it goes through VXLAN or GRE
tunnel and the physical NIC on the host supports hardware VXLAN/GRE
GSO offload (e.g. bnx2x and next-gen mlx4).
Two issues -
(VXLAN) VM traffic has SKB_GSO_DODGY and SKB_GSO_UDP_TUNNEL with
SKB_GSO_TCP/UDP set depending on the inner protocol. GSO header
integrity check fails in udp4_ufo_fragment if inner protocol is
TCP. Also gso_segs is calculated incorrectly using skb->len that
includes tunnel header. Fix: robust check should only be applied
to the inner packet.
(VXLAN & GRE) Once GSO header integrity check passes, NULL segs
is returned and the original skb is sent to hardware. However the
tunnel header is already pulled. Fix: tunnel header needs to be
restored so that hardware can perform GSO properly on the original
packet.

When the vlan code detects that the real device can do TX VLAN offloads
in hardware, it tries to arrange for the real device's header_ops to
be invoked directly.
But it does so illegally, by simply hooking the real device's
header_ops up to the VLAN device.
This doesn't work because we will end up invoking a set of header_ops
routines which expect a device type which matches the real device, but
will see a VLAN device instead.
Fix this by providing a pass-thru set of header_ops which will arrange
to pass the proper real device instead.
To facilitate this add a dev_rebuild_header(). There are
implementations which provide a ->cache and ->create but not a
->rebuild (f.e. PLIP). So we need a helper function just like
dev_hard_header() to avoid crashes.
Use this helper in the one existing place where the
header_ops->rebuild was being invoked, the neighbour code.

Pull networking fixes from David Miller:
"Some holiday bug fixes for 3.13... There is still one bug I'd like to
get fixed before 3.13-final.
The vlan code erroneously assignes the header ops of the underlying
real device to the VLAN device above it when the real device can
hardware offload VLAN handling. That's completely bogus because
header ops are tied to the device type, so they only expect to see a
'dev' argument compatible with their ops.
The fix is the have the VLAN code use a special set of header ops that
does the pass-thru correctly, by calling the underlying real device's
header ops but _also_ passing in the real device instead of the VLAN
device.
That fix is currently waiting some testing.

We've seen so many instances of people invoking skb_tx_timestamp()
after the device already has been given the packet, that it's worth
being a little bit more verbose and explicit in this comment.

ipgre_header_parse() needs to parse the tunnel's ip header and it
uses mac_header to locate the iphdr. This got broken when gre tunneling
was refactored as mac_header is no longer updated to point to iphdr.
Introduce skb_pop_mac_header() helper to do the mac_header assignment
and use it in ipgre_rcv() to fix msg_name parsing.

When CONFIG_DEBUG_FORCE_WEAK_PER_CPU or CONFIG_ARCH_NEEDS_WEAK_PER_CPU
is set, DEFINE_PER_CPU() explodes into cryptic series of definitions
to still allow using "static" for percpu variables while keeping all
per-cpu symbols unique in the kernel image which is required for weak
symbols. This ultimately converts the actual symbol to global whether
DEFINE_PER_CPU() is prefixed with static or not.
Unfortunately, the macro forgot to add explicit extern declartion of
the actual symbol ending up defining global symbol without preceding
declaration for static definitions which naturally don't have matching
DECLARE_PER_CPU(). The only ill effect is triggering of the following
warnings.
fs/inode.c:74:8: warning: symbol 'nr_inodes' was not declared. Should it be static?
fs/inode.c:75:8: warning: symbol 'nr_unused' was not declared. Should it be static?
Fix it by adding extern declaration in the DEFINE_PER_CPU() macro.

Pull libata fixes from Tejun Heo:
"There's one interseting commit - "libata, freezer: avoid block device
removal while system is frozen". It's an ugly hack working around a
deadlock condition between driver core resume and block layer device
removal paths through freezer which was made more reproducible by
writeback being converted to workqueue some releases ago. The bug has
nothing to do with libata but it's just an workaround which is easy to
backport. After discussion, Rafael and I seem to agree that we don't
really need kernel freezables - both kthread and workqueue. There are
few specific workqueues which constitute PM operations and require
freezing, which will be converted to use workqueue_set_max_active()
instead. All other kernel freezer uses are planned to be removed,
followed by the removal of kthread and workqueue freezer support,
hopefully.
Others are device-specific fixes. The most notable is the addition of
NO_NCQ_TRIM which is used to disable queued TRIM commands to Micro
M500 SSDs which otherwise suffers data corruption"

Certain drives cannot handle queued TRIM commands properly, even
though support is indicated in the IDENTIFY DEVICE buffer. This patch
allows for disabling the commands for the affected drives and apply it
to the Micron/Crucial M500 SSDs which exhibit incorrect protocol
behavior when issued queued TRIM commands, which could lead to silent
data corruption.

The arbitrary restriction on page counts offered by the core
migrate_page_move_mapping() code results in rather suspicious looking
fiddling with page reference counts in the aio_migratepage() operation.
To fix this, make migrate_page_move_mapping() take an extra_count parameter
that allows aio to tell the code about its own reference count on the page
being migrated.
While cleaning up aio_migratepage(), make it validate that the old page
being passed in is actually what aio_migratepage() expects to prevent
misbehaviour in the case of races.
