Fix potential out-of-bounds write to urb->transfer_buffer
usbip handles network communication directly in the kernel. When receiving a
packet from its peer, usbip code parses headers according to protocol. As
part of this parsing urb->actual_length is filled. Since the input for
urb->actual_length comes from the network, it should be treated as untrusted.
Any entity controlling the network may put any value in the input and the
preallocated urb->transfer_buffer may not be large enough to hold the data.
Thus, the malicious entity is able to write arbitrary data to kernel memory.

When using asynchronous read or write operations on the USB endpoints the
issuer of the IO request is notified by calling the ki_complete() callback
of the submitted kiocb when the URB has been completed.
Calling this ki_complete() callback will free kiocb. Make sure that the
structure is no longer accessed beyond that point, otherwise undefined
behaviour might occur.

On BXT platform Host Controller and Device Controller figure as
same PCI device but with different device function. HCD should
not pass data to Device Controller but only to Host Controllers.
Checking if companion device is Host Controller, otherwise skip.

PCI hotpluggable xhci controllers such as some Alpine Ridge solutions will
remove the xhci controller from the PCI bus when the last USB device is
disconnected.
Add a flag to indicate that the host is being removed to avoid queueing
configure_endpoint commands for the dropped endpoints.
For PCI hotplugged controllers this will prevent 5 second command timeouts
For static xhci controllers the configure_endpoint command is not needed
in the removal case as everything will be returned, freed, and the
controller is reset.
For now the flag is only set for PCI connected host controllers.

Give USB3 devices a better chance to enumerate at USB 3 speeds if
they are connected to a suspended host.
Solves an issue with NEC uPD720200 host hanging when partially
enumerating a USB3 device as USB2 after host controller runtime resume.

When entering ERROR_RECOVERY, the forced disconnect should
be processed first by queuing the work function to handle it
in the usual way. The required minimum delay should then be
done after this, before restoring the port to dual-role, and
thereby re-establishing the source/sink connection.

queue_delayed_work(), which is based on jiffies, is not
appropriate for accurate timing, such as timeouts needing
resolution on the order of milliseconds. Instead introduce
an hrtimer which upon timeout will call regular queue_work().
Redefine timeout values based on the PD spec minimum amounts
as these are generally the minimum time the policy engine
needs to wait before peforming the next action.
msleep() is also jiffies-based, so replace with usleep_range()
for the same reason. Further, PE_SRC_TRANSITION_SUPPLY can
simply be inlined as part of the PE_SRC_NEGOTIATE_CAPABILITY state
using an in-place sleep.

The QPNP PD PHY supports BIST modes as required by the USB PD
specification. Specifically for BMC signaling, upon reception
of BIST requests to enter Carrier Mode 2 or Test Data Mode,
place the PHY in the appropriate test mode when receiving
these messages in the RX handler.
While at it, remove the unnecessary kmalloc in the RX handler
as the temporary 32-byte buffer can be placed on the stack.

Add explicit write memory barrier before turning off QMP PHY clock to
make sure previous POWER DOWN operation (in disconnect case) or
putting PHY into autonomous mode (in suspend case) is completed.
Similar way add write memory barrier after performing POWER UP
operation (in connect case) or disabling PHY autonomous mode (in
resume case).

Currently when selecting a PDO the request object is specifying
900mA as the operating current, which is suboptimal if the
supply is capable of more. Instead, simply request for the
maximum available. In the case where a PDO does not meet the
minimum current, set the capability mismatch flag and indicate
the required amount via the maximum field in the request.

GDSC driver is removing control for USB memory power states.
DWC driver need to explicitly call clk_set_flag() API to turn
off both memory core and periphery upon vbus off. When core
clock is turned on both of them will be turn on automatically.

There is a possibility of tasklet bottom half handler racing
with dwc3_msm_suspend(). As a result before disabling the dwc3
irq, dwc3 interrupts are fired and once the suspend routine
disables the clocks bottom half handler gets a chance to run
and tries to access dwc3 register resulting into un-clocked
register access. Since dwc3 interrupt is already running in
threaded irq context, interrupt can be directly handled in
same context and avoid this race.

The policy engine should only request for a specific current
amount when establishing an explicit contract. When performing
a hard reset, suspend this by requesting 0 and only if it
was non-zero before, otherwise requesting the maximum may
invalidate the default current for non-PD cases.

The charger drivers now handle setting of this property without
input from USB gadget other than the negotiated current draw
amount (set via POWER_SUPPLY_PROP_CURRENT_MAX). Remove setting
of POWER_SUPPLY_PROP_ONLINE which helps simplify the
dwc3_msm_gadget_vbus_draw() function a bit.

The gadget mode notification to draw a specific current may
occur in context with interrupts disabled, however the charger
driver's implementation of power_supply_set_property() for
POWER_SUPPLY_CURRENT_MAX currently uses a mutex. Since
sleeping is disallowed in this case, instead schedule a
work function to do the call.

Driver allocates notify request in function bind() callback
but frees it in function disable() callback. Upon composition
switch adb daemon gets restarted causing function bind/unbind
to happen twice. This results into allocating notify request
memory twice and only freeing it once either upon disconnect
or next composition switch. Fix the issue freeing memory in
function unbind callback.

TRB's buffer related memory is allocated as part of gsi_bind() for each
USB GSI interface if it is part of USB compsition (i.e. USB multi config
composition). This memory is never de-allocated (i.e. USB cable
disconnect case) until USB composition switch. Also only one USB tethered
GSI interface is used out of mult-config based on selection of USB config
by USB host. Hence allocate TRB's buffer related memory after set_alt(1)
from ipa_work_handler() and de-allocate same from set_alt(0) OR USB
composition switch OR USB bus suspend with remote wakeup disable case.

Currently USB GSI function driver increments USB gadget device's
pm usage count on set_alt() and decrements on USB bus suspend or
USB cable disconnect case. Current code is not decrementing this
usage count when sm_state is STATE_INITIALIZED and USB cable is
disconnected (i.e. event EVT_DISCONNECTED posted). In this case,
USB doesn't go into low power mode although USB cable is disconnected.
Hence to handle this case make sure to increment USB device's PM
usage count from state machine instead of set_alt().
Also fix one of bug where state machine is peeking event but it is
not deleting same event from queue which shall result into using
same event multiple time in different states.
Use usb_gadget_autopm_get() instead of usb_gadget_autopm_get_noresume()
as noresume variant only increment usage count which is not sufficient
condition to prevent USB to go into LPM. usb_gadget_autopm_get()
increments both usage count and child count.

Upon usb composition switch there is possibility of ep0 file
release happening after gadget driver bind. In case of composition
switch from adb to a non-adb composition gadget will never gets
bound again resulting into failure of usb device enumeration. Fix
this issue by checking FFS_FL_BOUND flag and avoid extra
gadget driver unbind if it is already done as part of composition
switch.

Synchronization is required because bind/unbind can happen in two
different contexts. For example, function unbind due to

Driver is using "qcom,emu-init-seq" for finding length of property
but using "qcom,qemu-init-seq" instead of "qcom,emu-init-seq" to
get passed values from device tree. With this, length of property
comes as expected but values are not received. This results into
programming zeros as values with offset as zero instead of expected
<value, offset> passed through devicetree.

Upon usb disconnect or composition switch notify ep request buffer
as well as the request are freed. This is racing with user space
ioctl on character device trying to send the notification to USB
host. As a result notify ep request buffer is being dereferenced
causing NULL pointer dereference. Fix the issue by checking
is_connected flag by taking port lock before accessing notify
ep request and request buffer.

This patch fixes an issue that usbhsg_queue_done() may cause kernel
panic when dma callback is running and usb_ep_disable() is called
by interrupt handler. (Especially, we can reproduce this issue using
g_audio with usb-dmac driver.)
For example of a flow:
usbhsf_dma_complete (on tasklet)
--> usbhsf_pkt_handler (on tasklet)
--> usbhsg_queue_done (on tasklet)
*** interrupt happened and usb_ep_disable() is called ***
--> usbhsg_queue_pop (on tasklet)
Then, oops happened.

This patch adds a code to surely disable TX IRQ of the pipe before
starting TX DMAC transfer. Otherwise, a lot of unnecessary TX IRQs
may happen in rare cases when DMAC is used.

Commit 64d513ac31bd ("scsi: use host wide tags by default") causes
the SCSI core to queue more commands then we can handle on devices with
multiple LUNs, limit the queue depth at the scsi-host level instead of
per slave to fix this.
