xhci_stop_device() calls xhci_queue_stop_endpoint() multiple times
without checking the return value. xhci_queue_stop_endpoint() can
return error if the HC is already halted or unable to queue commands.
This can cause a deadlock condition as xhci_stop_device() would
end up waiting indefinitely for a completion for the command that
didn't get queued. Fix this by checking the return value and bailing
out of xhci_stop_device() in case of error. This patch happens to fix
potential memory leaks of the allocated command structures as well.

Currently our driver is notifying ETIMEDOUT to PMIC
incase of floating charger. Consider if we are inserting
an SDP charger slowly that time also we will detect
it as floating charger then also we are notifying
ETIMEDOUT irrespective of current that is notified from
gadget driver. Hence notify charging current properly
if enumeration happens even in case of
floating charger so that PMIC can update real charger
type to SDP

For improved performance, move RX handling from a thread to
a hard IRQ handler. This helps to decrease the chance of missing
incoming back-to-back messages (e.g. Accept -> PS_Ready following
a successful power request) due to the pending RX token in case
the IRQ thread is delayed due to high system load. policy_engine
needs to perform the buffer allocation with the GFP_ATOMIC flag.

Having this flag was causing PD TX messages to incorrectly
get blocked in the case where the policy engine state machine
worker got scheduled to run in the RX IRQ, but before the
handler exits and clears the flag. This caused false errors
that an RX message is pending. Remove usage of this flag as
it was doing more harm than good.

When sending/receiving a hard reset in source mode, the
PE_SRC_TRANSITION_TO_DEFAULT state turns off VBUS/VCONN and
sleeps for 750ms. During this fairly long period any Type-C
event such as cable disconnect or reconnect won't get
processed as the workqueue is blocked by this wait condition.
This can lead to incorrect state handling, for example when
a sink is removed and a source is attached; the state machine
will think a sink is still attached. Fix this by removing
the msleep() and set a timer to restart the state machine
to PE_UNKNOWN which will turn VBUS/VCONN back on and start
again in PE_SRC_STARTUP. This frees up the workqueue to
process any potential incoming events in the meantime.

Entering SNK_Ready or SRC_Ready state from usbpd_set_state()
is somewhat terminal as it is the end state of a contract
negotiation. If there was a previous pending VDM message
waiting to be sent, for example the start of a Discovery which
was queued prior to a PR Swap, it would not get sent. Check for
this and enqueue usbpd_sm once again so that it could be sent
out. Otherwise, start the Discovery process in case we are
in DFP mode, and do that for SNK_Ready as well.

If c->cdev->use_os_string flag is not set,
don't need to invoke ffs_do_os_descs() in _ffs_func_bind.
So uninitialized ext_compat_id pointer won't be accessed by
__ffs_func_bind_do_os_desc to cause kernel panic.

Currently GSI driver is clearing Doorbell to GSI in gsi_suspend(),
when it receives function suspend and notifying suspend to GSI in work
queue context. In ipa_suspend_work_handler(), driver is first checking
whether GSI wrapper is ready to suspend before doing xdci_suspend(). This
check fails if function resume received before ipa_suspend_work_handler()
gets chance to run. Due to this, state machine still stays at CONNECTED
state. When function resume is received, state machine is not handling
resume event and results in not ringing Doorbell to GSI. This eventually
causes data stall. Hence fix the issue by ringing Doorbell to GSI if in
case check for suspend fails in ipa_suspend_work_handler.

According to the databook ep0 should be in setup
phase during reset. If host issues reset between
control transfers, ep0 will be in an invalid state.
Fix this my issuing stall and restart on ep0 if it
is not in setup phase.

Some userspace modules expect USB driver to send event to userspace, when
usb device enumeration failure with device descriptor read errors.
Add USB_DEVICE_ERROR uevent in the USB driver to explicitly send the ERROR
uevent for such case.

In the case of ffs_free_inst() called, whole ffs_dev structure is
freed. Userspace related API do not check if ffs_dev is freed or
not.
If ffs endpoint is opened by userspace, ffs_free_inst() is executed,
mark inst_exist to false but do not free instance structures until
ffs_data is freed.
Besides, ffs_data is allocated in ffs_fs_mount() while opts->dev
is allocated when ffs instance created. And opts->dev will
be freed when ffs instance freed.
If ffs instance is freed and created once, opts->dev is allocated
to new memory, but since ffs_fs_mount() won't be called in this
case, new opts->dev miss the ffs_data address and
ffs_data->private_data still point to old opts->dev address which
is already freed.
So new allocated opts->dev need to initialize opts->dev->ffs_data,
and ffs_private_data also need to update new allocated opts->dev
address.

Currently driver is passing generic string as part of string descriptor
for all supported qdss channels. Instead pass channel specific string
because USB host may represent the enumerated device using string passed
in usb string descriptor.

The sunxi musb has a bug where sometimes it will generate a babble
error on device disconnect instead of a disconnect IRQ. When this
happens the musb controller switches from host mode to device mode
(it clears MUSB_DEVCTL_HM/MUSB_DEVCTL_SESSION and sets
MUSB_DEVCTL_BDEVICE) and gets stuck in this state.
The babble error is misdetected as a bus reset because MUSB_DEVCTL_HM
was cleared.
To fix this, use is_host_active() rather than (devctl & MUSB_DEVCTL_HM)
to detect babble error so that sunxi musb babble recovery can handle it
by restoring the mode. This information is provided by the driver logic
and does not rely on register contents.

This fixes a kernel oops when unloading the driver due to usb_put_phy
being called after usb_phy_generic_unregister when the device is
detached. Calling usb_phy_generic_unregister causes x->dev->driver to
be NULL in usb_put_phy and results in a NULL pointer dereference.

If the connect status change is set during reset signaling, but
the status remains connected just retry port reset.
This solves an issue with connecting a 90W HP Thunderbolt 3 dock
with a Lenovo Carbon x1 (5th generation) which causes a 30min loop
of a high speed device being re-discovererd before usb ports starts
working.

This is caused by a high speed device that doesn't successfully go to the
enabled state after the second port reset. Instead the connection bounces
(connected, with connect status change), bailing out completely from
enumeration just to restart from scratch.

This keyboard doesn't implement Get String descriptors properly even
though string indexes are valid. What happens is that when requesting
for the String descriptor, the device disconnects and
reconnects. Without this quirk, this loop will continue forever.

Andrey used the syzkaller fuzzer to find an out-of-bounds memory
access in usb_get_bos_descriptor(). The code wasn't checking that the
next usb_dev_cap_header structure could fit into the remaining buffer
space.
This patch fixes the error and also reduces the bNumDeviceCaps field
in the header to match the actual number of capabilities found, in
cases where there are fewer than expected.

Taking the uurb->buffer_length userspace passes in as a maximum for the
actual urbs transfer_buffer_length causes 2 serious issues:
1) It breaks isochronous support for all userspace apps using libusb,
as existing libusb versions pass in 0 for uurb->buffer_length,
relying on the kernel using the lenghts of the usbdevfs_iso_packet_desc
descriptors passed in added together as buffer length.
This for example causes redirection of USB audio and Webcam's into
virtual machines using qemu-kvm to no longer work. This is a userspace
ABI break and as such must be reverted.
Note that the original commit does not protect other users / the
kernels memory, it only stops the userspace process making the call
from shooting itself in the foot.
2) It may cause the kernel to program host controllers to DMA over random
memory. Just as the devio code used to only look at the iso_packet_desc
lenghts, the host drivers do the same, relying on the submitter of the
urbs to make sure the entire buffer is large enough and not checking
transfer_buffer_length.
But the "USB: devio: Don't corrupt user memory" commit now takes the
userspace provided uurb->buffer_length for the buffer-size while copying
over the user-provided iso_packet_desc lengths 1:1, allowing the user
to specify a small buffer size while programming the host controller to
dma a lot more data.
(Atleast the ohci, uhci, xhci and fhci drivers do not check
transfer_buffer_length for isoc transfers.)
This reverts commit fa1ed74eb1c2 ("USB: devio: Don't corrupt user memory")
fixing both these issues.

Cuurently as a part of start host, after the block reset we
are performing core init and gadget restart of the dwc3
controller. This causes crash for host only controller where
ep0 and ep1 have not been initialised but are being
dereferenced. Also, HW reinitialisation is not required for
start or restart host routine.

Windows XP host takes upto 6 seconds to enumerate RNDIS composition when
RNDIS (tethering) is enabled while in MTP/PTP composition. This results in
Userpsace Tethering enable timeout (1 second) causing RNDIS enumeration
failure and falls back to default composition. Workaround the issue by not
sending disconnect event to userspace on composition switch (MTP/PTP to
RNDIS). Send the disconnect event from USB bus reset so that userspace
enable timer starts from bus reset.

Make sure to reset the USB-console port pointer when console setup fails
in order to avoid having the struct usb_serial be prematurely freed by
the console code when the device is later disconnected.
