Reimplement interface masking using device flags stored directly in the
device-id table. This will make it easier to add and maintain device-id
entries by using a more compact and readable notation compared to the
current implementation (which manages pairs of masks in separate
blacklist structs).
Two convenience macros are used to flag an interface as either reserved
or as not supporting modem-control requests:

For now, we limit the highest maskable interface number to seven, which
allows for (up to 16) additional device flags to be added later should
need arise.
Note that this will likely need to be backported to stable in order to
make future device-id backports more manageable.

Some non-compliant high-speed USB devices have bulk endpoints with a
1024-byte maxpacket size. Although such endpoints don't work with
xHCI host controllers, they do work with EHCI controllers. We used to
accept these invalid sizes (with a warning), but we no longer do
because of an unintentional change introduced by commit aed9d65ac327
("USB: validate wMaxPacketValue entries in endpoint descriptors").
This patch restores the old behavior, so that people with these
peculiar devices can use them without patching their kernels by hand.

If we get an invalid device configuration from a palm 3 type device, we
might incorrectly parse things, and we have the potential to crash in
"interesting" ways.
Fix this up by verifying the size of the configuration passed to us by
the device, and only if it is correct, will we handle it.
Note that this also fixes an information leak of slab data.

The Quectel EP06 is a Cat. 6 LTE modem, and the interface mapping is as
follows:
0: Diag
1: NMEA
2: AT
3: Modem
Interface 4 is QMI and interface 5 is ADB, so they are blacklisted.
This patch should also be considered for -stable. The QMI-patch for this
modem is already in the -stable-queue.

USB QDSS function driver is already registered as module and having init
and exit APIs. DECLARE_USB_FUNCTION_INIT() adds additional module entries
for USB QDSS function driver. This results into seeing error as
"usb_qdss_init: failed to register diag -17" when usb_function_register()
is called 2nd time. Hence fix this issue by using DECLARE_USB_FUNCTION()
instead of DECLARE_USB_FUNCTION_INIT() API.

On chromebooks we depend on wakeup count to identify the wakeup source.
But currently USB devices do not increment the wakeup count when they
trigger the remote wake. This patch addresses the same.
Resume condition is reported differently on USB 2.0 and USB 3.0 devices.
On USB 2.0 devices, a wake capable device, if wake enabled, drives
resume signal to indicate a remote wake (USB 2.0 spec section 7.1.7.7).
The upstream facing port then sets C_PORT_SUSPEND bit and reports a
port change event (USB 2.0 spec section 11.24.2.7.2.3). Thus if a port
has resumed before driving the resume signal from the host and
C_PORT_SUSPEND is set, then the device attached to the given port might
be the reason for the last system wakeup. Increment the wakeup count for
the same.
On USB 3.0 devices, a function may signal that it wants to exit from device
suspend by sending a Function Wake Device Notification to the host (USB3.0
spec section 8.5.6.4) Thus on receiving the Function Wake, increment the
wakeup count.

Arrow USB Blaster integrated on MAX1000 board uses the same vendor ID
(0x0403) and product ID (0x6010) as the "original" FTDI device.
This patch avoids picking up by ftdi_sio of the first interface of this
USB device. After that this device can be used by Arrow user-space JTAG
driver.

USB3 hubs don't support global suspend.
USB3 specification 10.10, Enhanced SuperSpeed hubs only support selective
suspend and resume, they do not support global suspend/resume where the
hub downstream facing ports states are not affected.
When system enters hibernation it first enters freeze process where only
the root hub enters suspend, usb_port_suspend() is not called for other
devices, and suspend status flags are not set for them. Other devices are
expected to suspend globally. Some external USB3 hubs will suspend the
downstream facing port at global suspend. These devices won't be resumed
at thaw as the suspend status flag is not set.
A USB3 removable hard disk connected through a USB3 hub that won't resume
at thaw will fail to synchronize SCSI cache, return “cmd cmplt err -71”
error, and needs a 60 seconds timeout which causing system hang for 60s
before the USB host reset the port for the USB3 removable hard disk to
recover.
Fix this by always calling usb_port_suspend() during freeze for USB3
devices.

Pixel 2 field testers reported that when they tried to reboot their
phones with some USB devices plugged in, the reboot would get wedged and
eventually trigger watchdog reset. Once the Pixel kernel team found a
reliable repro case, they narrowed it down to this commit's 4.4.y
backport. Reverting the change made the issue go away.

In the ene_usb6250 sub-driver for usb-storage, the SCSI residue is not
reported correctly. The residue is initialized to 0, but this value
is overwritten whenever the driver sends firmware to the card reader
before performing the current command. As a result, a valid READ or
WRITE operation appears to have failed, causing the SCSI core to retry
the command multiple times and eventually fail.
This patch fixes the problem by resetting the SCSI residue to 0 after
sending firmware to the device.

In the ene_usb6250 sub-driver for usb-storage, the ene_transport()
routine is supposed to initialize the driver before executing the
current command, if the initialization has not already been performed.
However, a bug in the routine causes it to skip the command after
doing the initialization. Also, the routine does not return an
appropriate error code if either the initialization or the command
fails.
As a result of the first bug, the first command (a SCSI INQUIRY) is
not carried out. The results can be seen in the system log, in the
form of a warning message and empty or garbage INQUIRY data:

If ci_hdrc_host_init() or ci_hdrc_gadget_init() returns error and the
error != -ENXIO, as Peter pointed out, "it stands for initialization
for host or gadget has failed", so we'd better return failure rather
continue.
And before destroying the otg, i.e ci_hdrc_otg_destroy(ci), we should
also check ci->roles[CI_ROLE_GADGET].

Function devm_clk_get() returns an ERR_PTR when it fails. However, in
function kdwc3_probe(), its return value is not checked, which may
result in a bad memory access bug. This patch fixes the bug.

ffs can be used by many drivers, ADB can use it and MTP can
use it. Any other generic function can use it.
So we continue on the single instance patch, provide
multi-instance fix for the use-after-free issue.

Currently as a part of host cable connection, we are notifying
cable connect to phy driver first and then resuming the PHY. This
leads to bus resume case running instead of the cable connect
case resume, hence the phy driver does not vote for the LDOs. As
a result, PHY goes into bad state. Fix this by first resuming PHY
so that cable connect case resume runs.

In the earlier commit dad3f793f20f ("usb: dwc2: Make sure we
disconnect the gadget state"), I was trying to fix up the
fact that we somehow weren't disconnecting the gadget state,
so that when the OTG port was plugged in the second time we
would get warnings about the state tracking being wrong.
(This seems to be due to a quirk of the HiKey board where
we do not ever get any otg interrupts, particularly the session
end detected signal. Instead we only see status change
interrupt.)
The fix there was somewhat simple, as it just made sure to
call dwc2_hsotg_disconnect() before we connected things up
in OTG mode, ensuring the state handling didn't throw errors.
But in looking at a different issue I was seeing with UDC
state handling, I realized that it would be much better
to call dwc2_hsotg_disconnect when we get the state change
signal moving to host mode.
Thus, this patch removes the earlier disconnect call I added
and moves it (and the needed locking) to the host mode
transition.

When we unlock our spinlock to copy data to user we may get
disabled by USB host and free the whole list of completed out
requests including the one from which we are copying the data
to user memory.
To prevent from this let's remove our working element from
the list and place it back only if there is sth left when we
finish with it.

Using usb_ep_align() makes sure that the buffer size for OUT endpoints is
always aligned with wMaxPacketSize (512 usually). This makes sure
that no buffer has the wrong size, which can cause nasty bugs.

Length of buffers should be of type size_t whenever possible. Altough
recommended, this change has no real practical change, unless a driver has a
uses a huge or negative buffer size - it might help find these bugs.
