Following a command abort or device reset, ipr's EH handlers wait for
the commands getting aborted to get sent back from the adapter prior to
returning from the EH handler. This fixes up some cases where the
completion handler was not getting called, which would have resulted in
the EH thread waiting until it timed out, greatly extending EH time.

This patch fixes NULL pointer crash due to active timer running for abort
IOCB.
From crash dump analysis it was discoverd that get_next_timer_interrupt()

In function ufshcd_add_cmd_trace(), cmd_id entry is
not initialized when CONFIG_SCSI_UFSHCD_CMD_LOGGING
is disabled and resulting in compilation errors.This
change initializes cmd_id to fix compilation error.

In function ufs_qcom_testbus_config, offset variable is
declared but not initialized. Switch case can take the
default case and use the uninitialized offset variable.
This change fixes the uninitialized use of offset.

When an I/O is returned with an srb_status of SRB_STATUS_INVALID_LUN
which has zero good_bytes it must be assigned an error. Otherwise the
I/O will be continuously requeued and will cause a deadlock in the case
where disks are being hot added and removed. sd_probe_async will wait
forever for its I/O to complete while holding scsi_sd_probe_domain.
Also returning the default error of DID_TARGET_FAILURE causes multipath
to not retry the I/O resulting in applications receiving I/O errors
before a failover can occur.

The fcp_rsp_info structure as defined in the FC spec has an initial 3
bytes reserved field. The ibmvfc driver mistakenly defined this field as
4 bytes resulting in the rsp_code field being defined in what should be
the start of the second reserved field and thus always being reported as
zero by the driver.
Ideally, we should wire ibmvfc up with libfc for the sake of code
deduplication, and ease of maintaining standardized structures in a
single place. However, for now simply fixup the definition in ibmvfc for
backporting to distros on older kernels. Wiring up with libfc will be
done in a followup patch.

Function ufshcd_hba_exit might be called to destroy clk scaling work queue
if failure happends in function ufshcd_init before clk gating work queue
is created. This change fixs it by skipping destroying clk scaling work
queue if it is NULL.

The initio driver has for many years had two copies of the
same module device table. One of them is also used for registering
the other driver, the other one is entirely useless after the
large scale cleanup that Alan Cox did back in 2007.
The compiler warns about this whenever the driver is built-in:

This removes the extraneous table and the warning.

The mvumi scsi hides the references to its suspend/resume functions in
an #ifdef but does not hide the implementation the same way:

The fdomain SCSI host driver is one of the last remaining drivers that
manually search the PCI bus using pci_get_device rather than registering
a pci_driver instance.
This means the module device table is unused when the driver is
built-in, and we get a warning about it:

The advansys probe function tries to handle both ISA and PCI cases, each
hidden in an #ifdef when unused. This leads to a warning indicating that

_vreg_ is being dereferenced before it is null checked, hence there is a
potential null pointer dereference.
Fix this by moving the pointer dereference after _vreg_ has been null
checked.
This issue was detected with the help of Coccinelle.

As part of the scsi EH path, aacraid performs a reinitialization of the
adapter, which encompass freeing resources and IRQs, NULLifying lots of
pointers, and then initialize it all over again. We've identified a
problem during the free IRQ portion of this path if CONFIG_DEBUG_SHIRQ
is enabled on kernel config file.
Happens that, in case this flag was set, right after free_irq()
effectively clears the interrupt, it checks if it was requested as
IRQF_SHARED. In positive case, it performs another call to the IRQ
handler on driver. Problem is: since aacraid currently free some
resources *before* freeing the IRQ, once free_irq() path calls the
handler again (due to CONFIG_DEBUG_SHIRQ), aacraid crashes due to NULL
pointer dereference with the following trace:

This patch prevents the crash by changing the order of the
deinitialization in this path of aacraid: first we clear the IRQ, then
we free other resources. No functional change intended.

The SCSI host byte should be shifted left by 16 in order to have
scsi_decide_disposition() do the right thing (.i.e. requeue the

The current io_latency_state structure includes entries for read
and write requests. There are special types of write commands
such as sync and discard (trim) commands, and the current
implementation is not general enough if we want to separate
latency histogram for such special commands.
This change makes io_latency_state structure request-type neutral.
It also changes to print the latency average.

Local Reject/Invalid RPI errors seen during discovery.
Temporary RPI cleanup was occurring regardless of SLI rev. It's only
necessary on SLI-4.
Adjust the test for whether cleanup is necessary.

Firmware update fails with: status x17 add_status x56 on the final write
If multiple DMA buffers are used for the download, some firmware revs
have difficulty with signatures and crcs split across the dma buffer
boundaries. Resolve by making all writes be a single 4k page in length.

Whenever an I/O for a RAID volume fails with IOCStatus
MPI2_IOCSTATUS_SCSI_IOC_TERMINATED and SCSIStatus equal to
(MPI2_SCSI_STATE_TERMINATED | MPI2_SCSI_STATE_NO_SCSI_STATUS) then
return the I/O to SCSI midlayer with "DID_RESET" (i.e. retry the IO
infinite times) set in the host byte.
Previously, the driver was completing the I/O with "DID_SOFT_ERROR"
which causes the I/O to be quickly retried. However, firmware needed
more time and hence I/Os were failing.

In case of connection reset Tx skb queue can have some skbs which are
not transmitted so purge Tx skb queue in release_offload_resources() to
avoid skb leak.

lpfc cannot establish connection with targets that send PRLI in P2P
configurations.
If lpfc rejects a PRLI that is sent from a target the target will not
resend and will reject the PRLI send from the initiator.

We could allocate less memory than intended because we do:
bfad->regdata = kzalloc(len << 2, GFP_KERNEL);
The shift can overflow leading to a crash. This is debugfs code so the
impact is very small. I fixed the network version of this in March with
commit 13e2d5187f6b ("bna: integer overflow bug in debugfs").
