This task tried to open the cdrom device, the sr_block_open function
acquired the global sr_mutex lock. The call to check_disk_change()
then saw an event flag indicating a possible media change and tried
to flush any cached data for the device.
As part of the flush, it tried to acquire the super_block->s_umount
lock associated with the cdrom device.
This was the same super_block as created and locked by the previous task.
The first task acquires the s_umount lock and then the sr_mutex_lock;
the second task acquires the sr_mutex_lock and then the s_umount lock.
This patch fixes the issue by moving check_disk_change() out of
cdrom_open() and let the caller take care of it.

We can't just use scsi_cd() to get the scsi_cd structure, we have
to grab a live reference to the device. For both callbacks, we're
not inside an open where we already hold a reference to the device.
This fixes device removal/addition under concurrent device access,
which otherwise could result in the below oops.

If the read-only flag is true on a SCSI disk, re-reading the partition
table sets the flag back to false.
To observe this bug, you can run:
1. blockdev --setro /dev/sda
2. blockdev --rereadpt /dev/sda
3. blockdev --getro /dev/sda
This commit reads the disk's old state and combines it with the device
disk-reported state rather than unconditionally marking it as RW.

The firmware event workqueue should not be marked as WQ_MEM_RECLAIM
as it's doesn't need to make forward progress under memory pressure.
In the current state it will result in a deadlock if the device had been
forcefully removed.

This is because aac_shutdown() does
struct Scsi_Host *shost = pci_get_drvdata(dev);
scsi_block_requests(shost);
and that assumes shost has been assigned with pci_set_drvdata().
However, pci_set_drvdata(pdev, shost) is done in aac_probe_one() far
after bailing out with error from calling the init function
((*aac_drivers[index].init)(aac)), and when the init function fails, no
error is returned from aac_probe_one() so PCI layer assumes there is
driver attached, and tries to shut it down later.
Fix it by returning error from aac_probe_one() when card-specific init
function fails.
This fixes reboot on my HP NetRAID-4M with dead battery.

Increase cmd_per_lun to allow more I/Os in progress per device,
particularly for NVMe's. The Hyper-V host side can handle the higher
count with no issues.

We wanted to exit the loop with "div" set to zero, but instead, if we
don't hit the break then "div" is -1 when we finish the loop. It leads
to an array underflow a few lines later.

When a request times out we set the io_req flag BNX2FC_FLAG_IO_COMPL so
that if a subsequent completion comes in on that task ID we will ignore
it. The issue is that in the check for this flag there is a missing
return so we will continue to process a request which may have already
been returned to the ownership of the SCSI layer. This can cause
unpredictable results.
Solution is to add in the missing return.

While testing with the ARM specific memset() macro removed, I ran into a
compiler warning that shows an old bug:

drivers/scsi/arm/fas216.c:2014:40: error: argument to 'sizeof' in 'memset' call is the same expression as the destination; did you mean to provide an explicit length? [-Werror=sizeof-pointer-memaccess]
It turns out that the definition of the scsi_cmd structure changed back
in linux-2.6.25, so now we clear only four bytes (sizeof(pointer))
instead of 96 (SCSI_SENSE_BUFFERSIZE). I did not check whether we
actually need to initialize the buffer here, but it's clear that if we
do it, we should use the correct size.

On auto platforms, it is observed that sdcard is not getting
detected using usb card reader as check_events callback was
removed. For usb card reader case, scsi check_events() callback
is required which sends test unit ready command to check if
underlying media state has changed. So instead of removing
check_events() completely, simply return for embedded storage
media like UFS.

When ata device doing EH, some commands still attached with tasks are
not passed to libata when abort failed or recover failed, so libata did
not handle these commands. After these commands done, sas task is freed,
but ata qc is not freed. This will cause ata qc leak and trigger a

If ata qc leaked too many, ata tag allocation will fail and io blocked
for ever.
As suggested by Dan Williams, defer ata device commands to libata and
merge sas_eh_finish_cmd() with sas_eh_defer_cmd(). libata will handle
ata qcs correctly after this.

The status of SAS PHY is in sas_phy->enabled. There is an issue that the
status of a remote SAS PHY may be initialized incorrectly: if disable
remote SAS PHY through sysfs interface (such as echo 0 >
/sys/class/sas_phy/phy-1:0:0/enable), then reboot the system, and we
will find the status of remote SAS PHY which is disabled before is
1 (cat /sys/class/sas_phy/phy-1:0:0/enable). But actually the status of
remote SAS PHY is disabled and the device attached is not found.
In SAS protocol, NEGOTIATED LOGICAL LINK RATE field of DISCOVER response
is 0x1 when remote SAS PHY is disabled. So initialize sas_phy->enabled
according to the value of NEGOTIATED LOGICAL LINK RATE field.

The intend purpose here was to goto out if smp_execute_task() returned

1. In IO path, setting of "ATA command pending" flag early before device
removal, invalid device handle etc., checks causes any new commands
to be always returned with SAM_STAT_BUSY and when the driver removes
the drive the SML issues SYNC Cache command and that command is
always returned with SAM_STAT_BUSY and thus making SYNC Cache command
to requeued.
2. If the driver gets an ATA PT command for a SATA drive then the driver
set "ATA command pending" flag in device specific data structure not
to allow any further commands until the ATA PT command is completed.
However, after setting the flag if the driver decides to return the
command back to upper layers without actually issuing to the firmware
(i.e., returns from qcmd failure return paths) then the corresponding
flag is not cleared and this prevents the driver from sending any new
commands to the drive.
This patch fixes above two issues by setting of "ATA command pending"
flag after checking for whether device deleted, invalid device handle,
device busy with task management. And by setting "ATA command pending"
flag to false in all of the qcmd failure return paths after setting the
flag.

If, for any reason, userland shuts down iscsi transport interfaces
before proper logouts - like when logging in to LUNs manually, without
logging out on server shutdown, or when automated scripts can't
umount/logout from logged LUNs - kernel will hang forever on its
sd_sync_cache() logic, after issuing the SYNCHRONIZE_CACHE cmd to all

This happens because iscsi_eh_cmd_timed_out(), the transport layer
timeout helper, would tell the queue timeout function (scsi_times_out)
to reset the request timer over and over, until the session state is
back to logged in state. Unfortunately, during server shutdown, this
might never happen again.
Other option would be "not to handle" the issue in the transport
layer. That would trigger the error handler logic, which would also need
the session state to be logged in again.
Best option, for such case, is to tell upper layers that the command was
handled during the transport layer error handler helper, marking it as
DID_NO_CONNECT, which will allow completion and inform about the
problem.
After the session was marked as ISCSI_STATE_FAILED, due to the first
timeout during the server shutdown phase, all subsequent cmds will fail
to be queued, allowing upper logic to fail faster.

If multiple tasks attempt to read the stats, it may happen that the
start_req_done completion is re-initialized while still being used by
another task, causing a list corruption.

If the list search in sg_get_rq_mark() fails to find a valid request, we
return a bogus element. This then can later lead to a GPF in
sg_remove_scat().
So don't return bogus Sg_requests in sg_get_rq_mark() but NULL in case
the list search doesn't find a valid request.

Commit da244654c66e ("[SCSI] mac_esp: fix for quadras with two esp
chips") added mac_scsi_esp_intr() to handle the IRQ lines from a pair of
on-board ESP chips (a normal shared IRQ did not work).
Proper mutual exclusion was missing from that patch. This patch fixes
race conditions between comparison and assignment of esp_chips[]
pointers.

There are two versions of a structure for queue creation and setup that the
driver shares with FW. The driver was only treating as version 0.
Verify WQ_CREATE with 128B WQEs in V0 and V1.
Code review of another bug showed the driver passing
128B WQEs and 8 pages in WQ CREATE and V0.
Code inspection/instrumentation showed that the driver
uses V0 in WQ_CREATE and if the caller passes queue->entry_size
128B, the driver sets the hdr_version to V1 so all is good.
When I tested the V1 WQ_CREATE, the mailbox failed causing
the driver to unload.

Passed through SCSI targets may have transfer limits which come from the
host SCSI controller or something on the host side other than the target
itself.
To make this work properly, the hypervisor can adjust the target's VPD
information to advertise these limits. But for that to work, the guest
has to look at the VPD pages, which we won't do by default if it is an
SPC-2 device, even if it does actually support it.
This adds a workaround to address this, forcing devices attached to a
virtio-scsi controller to always check the VPD pages. This is modelled
on a similar workaround for the storvsc (Hyper-V) SCSI controller,
although that exists for slightly different reasons.
A specific case which causes this is a volume from IBM's IPR RAID
controller (which presents as an SPC-2 device, although it does support
VPD) passed through with qemu's 'scsi-block' device.

Don't make any assumptions on the sg_io_hdr_t::dxfer_direction or the
sg_io_hdr_t::dxferp in order to determine if it is a valid request. The
only way we can check for bad requests is by checking if the length
exceeds 256M.

dxfer_len is an unsigned int and we always assign a value > 0 to it, so
it doesn't make any sense to check if it is < 0. We can't really check
dxferp as well as we have both NULL and not NULL cases in the possible
call paths.
So just return true for SG_DXFER_FROM_DEV transfer in
sg_is_valid_dxfer().

Since scsi_get_device_flags_keyed() callers do not check whether or not
the returned value is an error code, change that function such that it
returns a flags value even if the 'key' argument is invalid. Note:
since commit 28a0bc4120d3 ("scsi: sd: Implement blacklist option for
WRITE SAME w/ UNMAP") bit 31 is a valid device information flag so
checking whether bit 31 is set in the return value is not sufficient to
tell the difference between an error code and a flags value.

The commit 08024885a2a3 ("ses: Add power_status to SES device slot")
introduced the 'power_status' attribute to enclosure components and
the associated callbacks.
There are 2 callbacks available to get the power status of a device:
1) ses_get_power_status() for 'struct enclosure_component_callbacks'
2) get_component_power_status() for the sysfs device attribute
(these are available for kernel-space and user-space, respectively.)
However, despite both methods being available to get power status
on demand, that commit also introduced a call to get power status
in ses_enclosure_data_process().
This dramatically increased the total probe time for SCSI devices
on larger configurations, because ses_enclosure_data_process() is
called several times during the SCSI devices probe and loops over
the component devices (but that is another problem, another patch).
That results in a tremendous continuous hammering of SCSI Receive
Diagnostics commands to the enclosure-services device, which does
delay the total probe time for the SCSI devices __significantly__:
Originally, ~34 minutes on a system attached to ~170 disks:

Back to the commit discussion.. on the ses_get_power_status() call
introduced in ses_enclosure_data_process(): impact of removing it.
That may possibly be in place to initialize the power status value
on device probe. However, those 2 functions available to retrieve
that value _do_ automatically refresh/update it. So the potential
benefit would be a direct access of the 'power_status' field which
does not use the callbacks...
But the only reader of 'struct enclosure_component::power_status'
is the get_component_power_status() callback for sysfs attribute,
and it _does_ check for and call the .get_power_status callback,
(which indeed is defined and implemented by that commit), so the
power status value is, again, automatically updated.
So, the remaining potential for a direct/non-callback access to
the power_status attribute would be out-of-tree modules -- well,
for those, if they are for whatever reason interested in values
that are set during device probe and not up-to-date by the time
they need it.. well, that would be curious.
Well, to handle that more properly, set the initial power state
value to '-1' (i.e., uninitialized) instead of '1' (power 'on'),
and check for it in that callback which may do an direct access
to the field value _if_ a callback function is not defined.
