Unlike MSI, which is configured via registers in the MSI capability in
Configuration Space, MSI-X is configured via tables in Memory Space.
These MSI-X tables are mapped by a device BAR, and if no Memory Space
has been assigned to the BAR, MSI-X cannot be used.
Fail MSI-X setup if no space has been assigned for the BAR.
Previously, we ioremapped the MSI-X table even if the resource hadn't been
assigned. In this case, the resource address is undefined (and is often
zero), which may lead to warnings or oopses in this path:
pci_enable_msix
msix_capability_init
msix_map_region
ioremap_nocache
The PCI core sets resource flags to zero when it can't assign space for the
resource (see reset_resource()). There are also some cases where it sets
the IORESOURCE_UNSET flag, e.g., pci_reassigndev_resource_alignment(),
pci_assign_resource(), etc. So we must check for both cases.

This changes accesses from (in|out)_(8|le16|le32) to readX/writeX variants.
I believe these should be equivalent for PCI config space accesses, but
confirmation would be nice.

Convert the sa1100 nanoengine PCI driver to use the generic config access
functions.
Change accesses from __raw_readX/__raw_writeX to readX/writeX variants.
This removes the spinlock because it is unnecessary. The config read and
write functions are already protected with a spinlock.

Convert the integrator PCI driver to use the generic config access
functions.
This changes accesses from __raw_readX/__raw_writeX to readX/writeX
variants. The spinlock is removed because it is unnecessary. The config
read and write functions are already protected with a spinlock and no
access can occur during the .pre_init function.

Most if not all ARM PCI host controller device drivers either ignore the
domain field in the pci_sys_data structure or just increment it every time
a host controller is probed, using it as a domain counter.
Therefore, instead of relying on pci_sys_data to stash the domain number in
a standard location, ARM pcibios code can be moved to the newly introduced
generic PCI domains code, implemented in commits:
41e5c0f81d3e ("of/pci: Add pci_get_new_domain_nr() and of_get_pci_domain_nr()")
670ba0c8883b ("PCI: Add generic domain handling")
ARM code is made to select PCI_DOMAINS_GENERIC by default, which builds
core PCI code that assigns the domain number through the generic function:
void pci_bus_assign_domain_nr(...)
that relies on a DT property to define the domain number or falls back to a
counter according to a predefined logic; its usage replaces the current
domain assignment code in PCI host controllers present in the kernel.

On cns3xxx platforms the PCI controller probing code relies on an
artificial dependency on the domain number to look-up the internal data
structures.
This patch reworks the host controller control data structure and adds a
domain equivalent field named port in it so that the dependency on
pci_sys_data domain field can be eventually removed.

The current logic in arm64 pci_bus_assign_domain_nr() is flawed in that
depending on the host controller configuration for a platform and the
initialization sequence, core code may end up allocating PCI domain numbers
from both DT and the generic domain counter, which would result in PCI
domain allocation aliases/errors.
Fix the logic behind the PCI domain number assignment and move the
resulting code to the PCI core so the same domain allocation logic is used
on all platforms that select CONFIG_PCI_DOMAINS_GENERIC.

Ballooned pages are always used for grant maps which means the
original frame does not need to be saved in page->index nor restored
after the grant unmap.
This allows the workaround in netback for the conflicting use of the
(unionized) page->index and page->pfmemalloc to be removed.

The scratch frame mappings for ballooned pages and the m2p override
are broken. Remove them in preparation for replacing them with
simpler mechanisms that works.
The scratch pages did not ensure that the page was not in use. In
particular, the foreign page could still be in use by hardware. If
the guest reused the frame the hardware could read or write that
frame.
The m2p override did not handle the same frame being granted by two
different grant references. Trying an M2P override lookup in this
case is impossible.
With the m2p override removed, the grant map/unmap for the kernel
mappings (for x86 PV) can be easily batched in
set_foreign_p2m_mapping() and clear_foreign_p2m_mapping().

When unmapping grants, instead of converting the kernel map ops to
unmap ops on the fly, pre-populate the set of unmap ops.
This allows the grant unmap for the kernel mappings to be trivially
batched in the future.

In Dom0's the use of the TSC clocksource (whenever it is stable enough to
be used) instead of the Xen clocksource should not cause any issues, as
Dom0 VMs never live-migrated. The TSC clocksource is somewhat more
efficient than the Xen paravirtualised clocksource, thus it should have
higher rating.
This patch decreases the rating of the Xen clocksource in Dom0s to 275.
Which is half-way between the rating of the TSC clocksource (300) and the
hpet clocksource (250).

With Xen configured into the arm64 kernel, any driver allocating
DMA'able memory for PCI operations, must be GPL compatible, regardless
of its interaction with Xen. This patch relaxes the GPL requirement of
xen_dma_ops and its dependencies to allow open source drivers to be
compiled for the arm64 architecture.

virtio wants to read bitwise types from userspace using get_user. At the
moment this triggers sparse errors, since the value is passed through an
integer.

A message was using pr_info level output for a message including "ERROR"
which is not really a fatal error.
Remove the 'ERROR' from that message, use pr_warn loglevel and add the
function name to the output to give users a chance to find the culprit
in case the warning triggers.

PVR version code was missing in the cpu_ver_lookup table for the following
versions: 8.50.b 8.50.c 9.2 9.3
This caused /proc/cpuinfo to display "CPU-Ver: Unknown" for these versions.
This was detected and the patch tested with MicroBlaze version 8.50.c.
The other codes were taken from the Xilinx MicroBlaze Processor Reference
Guides UG081 (v14.7) and UG984 (v2014.1).

With compilers which follow the C99 standard (like modern versions of gcc and
clang), "extern inline" does the opposite thing from older versions of gcc
(emits code for an externally linkable version of the inline function).
"static inline" does the intended behavior in all cases instead.
